The KDx science team and advisors carefully review the literature to identify the best evidence for the genetic testing, biomarker analysis, and neuromuscular training programs that we offer.

Genetic Profile

 

Our genetic profile includes 58 genetic markers. The KDx science team selected them based on scientific studies that showed good evidence of being associated with the trait or condition of interest, and there were reasonable actions that one could take if a “risk” variant is found.  In the genetics world one of the most important measurements of scientific validity is the p-value. The p-value is a statistical measurement that describes the probability of occurrence of a specific outcome. Most genomic researchers have agreed that a p-value of 10-8 is the “significance threshold” they believe is most likely to be true when associating a genetic variant with a specific disease.  Associations with a p-value of 10-6  and 10-7 are considered borderline, and those with a higher p-value > 10-5 may have a higher false discovery rate (FDR). 

 We use a range of p-values to determine the level of evidence around a genetic variant and a trait. Because we are not providing a diagnosis of any disease, and our recommendations typically involve exercise, nutrition, and follow up blood testing with expert oversight, we use less strict criteria for our evidence than a clinical geneticist who may be using this data to diagnose significant disease, prescribe a medication or recommend invasive tests.  We monitor the literature, and if new studies are published, we update the evidence and modify the testing panel as appropriate. Our evidence ranking is:

  1. Excellent ( large studies across large populations, preferably mixed demographics, with p values range 10-5 to 10-8)
  2. Good(decent studies across appropriately sized populations, p values range 10-3 to 10-4)
  3. Developing (small studies,  p values >10-3)

 We focus primarily on variants with level 1 and 2 evidence. We exclude level 3 evidence in general, but we have included some level 3 variants that are associated with more than one trait, and at least one association is 2 or higher. We also include one level 3 with some promising initial results in a large study, but rank it as a 3 due to lack of replication.

 Calculating your Risks

We do not calculate out specific risks associated with the genetic markers. The particular genetic markers in our panel are considered “multi-factorial”, meaning that they are not the sole influence on the trait of interest. The traits we report on are also influenced by activity level, diet, training history, medical history, level of play, type of equipment used, surface on which the activity or sport is played. In other words, many variables work together to determine an individual’s risk.  It would be misleading to provide a risk estimate based on genetics alone.

 Laboratory

The laboratory we use for the genetic profile is Gene by Gene. They are CLIA certified and licensed in all states that require such licensing. They are accredited by the College of American Pathologists (CAP) and the American Association of Blood Banks (AABB). You can see more of their accreditations at https://www.genebygene.com/.

Biomarkers

 

The Kinetic Diagnostics science and medicine team developed a list of routine blood tests (biomarkers) that are of interest to athletes using the research literature, professional society recommendations and industry best practices. The evidence for each test varies based on the current state of the athlete. Current professional guidelines from the National Athletic Trainers Society and the American Association of Family Physicians state that no routine blood work is required or of benefit in evaluating an athlete in a pre-participation physical exam, unless symptoms or a family history that merit special testing is present. However, popular opinion in the sports community suggests that a “baseline test” to understand what is “normal” for an individual athlete could be useful should symptoms, like fatigue or performance drop off, occur. Once an athlete has experienced symptoms suggestive of over-reaching (OR) or over-training (OT), a variety of tests have been suggested through research and professional society consensus statements to be of value in identifying the source of the athlete’s issues and to determine a strategy to bring balance back to the training/recovery time ratio. In general, using our overall rating system below, the evidence overall for biomarker testing is a “2.”

The evidence ranking is:

1= Excellent: Randomized clinical trials (RCT), and or professional society guidelines with good evidence

2= Good: Some evidence based studies and or professional society recommendations based on good evidence and or expert consensus

3= Developing: Expert opinion with consensus across multiple organizations

  • Albumin, Serum: Albumin is a form of protein produced in the liver and circulates in the blood to help maintain fluid balance in the body. It contains the nutrients and proteins required to clot blood properly in the case of bleeding. An Albumin test helps determine if you bodies liver is functioning properly.
  • Alkaline Phosphatase, S: Alkaline Phosphatase is an enzyme found in body tissues in the liver, bone, and kidneys. Elevated levels can be signs of liver or bone disorders.
  • ALT (SGPT): Alanine aminotransferase is an enzyme found in the liver and kidney. In healthy individuals, ALT levels are low, but when the liver is damaged more ALT flows into the bloodstream.
  • Apolipoprotein B: Apolipoprotein B is a protein that helps in the metabolism of lipids by combining with them to transport them through the blood stream. Increased levels of Apolipoprotein B are associated with those who have high cholesterol and a higher risk of cardiovascular disease.
  • AST (SGOT): Aspartate aminotransferase is an enzyme an enzyme most commonly found in the heart and liver. When liver or muscle cells are injured, AST is released into the blood. A damaged liver can increase AST levels so the test is used to help detect liver damage that may be due to diseases, drugs, or alcohol. 
  • Bilirubin, Total: Bilirubin is a waste product of the breakdown of heme, a component of hemoglobin. Testing bilirubin levels helps diagnose conditions such as liver disease, hemolytic anemia, and bile duct blocks.
  • BUN: The blood urea nitrogen or BUN test is primarily used to evaluate kidney function. Elevated blood urea nitrogen levels could indicate that the kidneys are not filtering waste as expected.
  • BUN+Creatinine: Creatinine is the waste product that comes from the muscles breaking down creatine. The creatinine blood test is ordered along with the blood urea nitrogen (BUN) test to assess kidney function.
  • C-Reactive Protein, Quantitative: C- Reactive protein (CRP) is a protein made by the liver and released into the blood after tissue injury, an infection, or other inflammation. The test is used to detect inflammation as levels of CRP increase when there is inflammation in the body.
  • Calcium, Serum: A total calcium level is often measured as part of a routine health screening. A blood calcium test may help screen for a range of conditions relating to the bones, heart, nerves, kidneys, and teeth.
  • Carbon Dioxide, Total: Carbon Dioxide is in the blood in the form of Bicarbonate. It is a negatively charged ion that is excreted and reabsorbed by the kidneys. The body uses it to maintain an acid-base balance. Imbalances of bicarbonate can be a sign of kidney disease, acidosis, or metabolic alkalosis.
  • CBC/Diff: The complete blood count (CBC) with differential test for illnesses in the blood and can be an indicator of overall body health. The test provides information and measurements on the number and types of white blood cells, the number of red blood cells, the size of your red blood cells, hematocrit, hemoglobin, average red blood cell size, platelet count, and mean corpuscular hemoglobin.
  • Chloride, Serum: Chloride, a negatively charged ion, is an electrolyte that works with other electrolytes, like potassium and sodium, to regulate the amount of fluid in the body. It is usually consumed into the body through food and table salt. Increased levels in the blood is linked with dehydration, weakness, or kidney disease.
  • Cholesterol, Total: Cholesterol is a waxy fat like substance found in the blood. It is important to make hormones, vitamin D, and to process some foods. Too much can lead to clogged arteries and heart disease. In athletes, elevated cholesterol has been associated with other conditions such as inflammation and female athlete triad syndrome. 
  • Cortisol: Cortisol is a hormone that plays a part in the breaking down of proteins, lipids, and carbohydrates. It also works to help maintain blood pressure and regulate the body’s immune system. Cortisol testing is done to help diagnose Cushing syndrome, which can lead to weight gain and fatty deposits in the face and upper back area, and adrenal insufficiency.
  • Creatine Kinase,Total, Serum: Creatine Kinase is an enzyme found in the heart, brain, and skeletal muscle. When muscle is damaged increased amounts ofCK is released into the blood. The test is used to detect inflammation of muscles or muscle damage. 
  • Estradiol: Estradiol (E2) is produced in the ovaries in women and in the testicles in men. It is a primary component in reproductive organ development and function in women and bone growth in both sexes. Abnormal levels can be associated with tumors in the ovaries, testicles, or adrenal glands or the dysfunction of these glands.
  • Ferritin, Serum: Ferritin is a protein found inside the cells that store iron. Measuring ferritin levels allows doctors to test measure the amount of iron in the blood. Iron is an essential component for red blood cell production.
  • Folate (Folic Acid), Serum :Folate helps form red blood cells and produce DNA. Low levels of Folate in the blood indicates malnutrition and poor diet.
  • Glomerular Filtration Rate, Estimated: Glomerular Filtration Rate (GFR) testing measures kidney function by measuring the level of creatinine, a waste product that the kidneys filter, in the blood. In the kidney’s glomeruli are tiny filters that remove waste from the blood, but allow important compounds like proteins and blood cells though. Measuring the waste in the blood through a GFR test indicates kidney health.
  • Glucose, Serum: This is a fasting glucose (sugar) test (fasting blood glucose, FBG) which measures the level of glucose in the blood after fasting for at least 8 hours. This is often part of routine health screening, and can help identify when the body may not be processing glucose as expected.
  • HDL Cholesterol: High- Density Lipoprotein (HDL) Cholesterol is the “good” cholesterol, as it consists primarily of protein and a small amount of cholesterol. It carries excess cholesterol from tissues to the liver for disposal.  Low Levels of HDL Cholesterol shows a correlation with an increased risk of heart disease.
  • Hemoglobin A1c: Hemoglobin is the protein containing iron that is found in all red blood cells. Hemoglobin allows the red blood cells to bind to oxygen in the lungs and transport it throughout the body. Testing for hemoglobin shows a correlation to red blood cell production in the body.
  • Insulin: Insulin is a hormone that is important to the transportation and storage of glucose in the body. It is made and stored in the pancreas. Low levels of insulin are seen in diabetes and pancreatic disease cases, high levels are associated with obesity and Cushing syndrome.
  • Iron, Serum: Iron is an essential part of life. It helps form red blood cells and is a critical component of hemoglobin, which helps bind oxygen in the lungs and circulates it to other areas of the body. Low levels of iron leads to chronic fatigue, weakness, and headaches.
  • LDL Cholesterol (Direct): Low-density lipoprotein cholesterol (LDL-C), also known as the bad cholesterol, testing is used to predict a persons risk for the development of heart disease. Maintaining healthy levels of lipids in the body is important, but excess cholesterol like LDL-C can lead to deposits on the walls of blood vessels that can lead to further complications as more builds up. 
  • Lipoprotein Lp(a): The Lipoprotein Lp(a) the test measures Lp(a) in the blood in order to evaluate an individuals risk for developing cardiovascular disease. Lp(a) levels remain relatively consistent over a lifespan, so changes in Lp(a) levels are an indication of risks.
  • Potassium, Serum: Potassium is an electrolyte that is essential is cell metabolism and muscle function. Potassium works to regulate the amount of fluid in the body and simulates muscle contraction. These tests are ordered when evaluating high blood pressure and kidney disease.
  • Protein, Total, Serum: The total protein test measures the amount of albumin and globulin in the blood. Albumin proteins keep fluid from leaking out of blood vessels and Globulin proteins play an important role in your immune system. Elevated levels can be signs of viral infections or bone marrow disorders. Low protein levels can indicate liver or kidney disorder, or malnutrition.
  • Sodium, Serum: Sodium is an electrolyte that is important to for nerve and muscle function. These tests are ordered in routine lab tests to determine electrolyte imbalance. An electrolyte imbalance can be associated with weakness, hyponatremia, and hypernatremia.
  • Testosterone, Free: Testosterone is a hormone produced by the testicles in males and the adrenal glands. Testosterone stimulates male puberty and maintaining muscle mass. Testing for free testosterone represents the fraction that circulates the blood and is a good indicator of the bioactivity of testosterone. 
  • Thyroxine (T4): Thyroxine (T4) is a hormone produced by the thyroid and it plays an important role in growth and metabolism. Low T4 levels can indicate dietary issues like malnutrition, while high levels can indicate high protein levels in the blood. 
  • Transferrin Saturation:    This test is used in combination with other iron tests to evaluate iron metabolism, and can be useful for endurance athletes. It can assist in determining between anemia that is caused by iron deficiency or that associated with inflammation or chronic illness.
  • Triglycerides: Triglycerides are a form of fat and a major source of energy for the body. However, levels that are too high are associated with a risk of developing heart disease. Elevated levels can be caused by smoking, inactivity, and high calorie diets.
  • Triiodothyronine (T3) : Triidothyronine (T3) is a hormone produced by the thyroid. Low levels are associated with a slowed metabolism and can lead to weight gain, dry skin, and fatigue. Hi levels are associated with nervousness, weight loss, and shaky hands.
  • TSH: The thyroid simulating hormone (TSH) is produced in the pituitary gland and it works with the thyroid gland to make and release thyroid hormones into the blood stream. Abnormal levels link to a problem with the pituitary gland or thyroid gland. Signs of this can be seen in weakness, difficulty sleeping, dry skin, and menstrual irregularity in women.
  • Vitamin B12: Vitamin B12 is an essential component of bodily health and is necessary for red blood cell formation, repairing tissues, DNA synthesis. It is not produced in the body and is only supplied through diet. B12 deficiency can be a sign of poor nutrition or of other issues blocking the body from properly absorbing B12 in the intestines. 
  • Vitamin D, 25-Hydroxy: Vitamin D is a family of compounds that are essential for the formation and growth of teeth and bones. Two forms are found in the blood, 25-hydoxyvitamin D and 1,25dihydoxyvitamin D. Vitamin D can be absorbed by the sun or ingested through food. Testing vitamin D can help determine if bone related abnormalities are related to abnormal vitamin D levels.

Neuromuscular Training Programs

Our science and athletic training team developed training programs and injury prevention recommendations based on evidence using research literature, professional society recommendations and industry best practices.  Evidence was evaluated only for prevention of initial injury (see Table 1). Ranking of evidence could change if considered for treatment of injury or prevention of subsequent injuries. Ranking of evidence may change by sport (see Table 2).

Once the KDx training team develops a program, it is reviewed and approved by our sports medicine advisors. All training programs should be undertaken with guidance from a trained, certified professional to maximize safety and ensure that programs are customized, if appropriate, for the athlete.

KDxome

Exome sequencing is performed by GenebyGene (CLIA # 45D1102202, www.genebygene.com) and a variant call file (VCF) is signed out by the lab director, Connie Bormans, Ph.D FACMG. This is considered the final lab report.

This test was developed and its performance characteristics determined by Gene by Gene. The U.S. Food and Drug Administration has not approved or cleared this test; however, FDA clearance or approval is not currently required for clinical use. The results are not intended to be used as a means for clinical diagnosis, treatment or patient management decisions.

This test should be regarded as a screening test, not a diagnostic test. This test will not diagnose disease or guide treatment decisions.  Genetic variants identified must be confirmed by another testing method, preferably Sanger sequencing, before clinical action or decisions are made.

The VCF is uploaded into Opal, software provided by Omicia, Inc. Opal attaches information to the variants from the following publically available databases:

Variants for review are identified by filtering the gene lists, which represents 5,144 genes, according to the protocols described below. At least one genetic counselor reviews each variant. A physician board certified as a medical geneticist does a second review and approves the educational report. When the evidence is questionable or borderline, the genetic counselors and medical geneticists collaborate on a consensus. If no consensus is reached, the Medical Director makes the final decision about whether or not to report a variant. Kinetic Diagnostics does not use any special algorithms or combine multiple variants together for an interpretation, which is called a haplotype. This limits information provided for some sections, such as Drug Response and Immune System as some informative information may come from haplotypes. Discuss any concerns about this limitation with your genetic counselor or clinical team. This test will not identify variants that are found in the current reference sequence. An example is Factor V Leiden, a common variant associated with thrombophilia. This test 

ACMG Section

Filter for variants found in <5% of the population, are found in a public database that collates available research, is predicted to change the protein, and meets the 2015 guidelines of the ACMG for reporting.

Carrier Screening Section

Filter for variants found in <5% of the population, are found in a public database that collates available research, is predicted to change the protein, and has significant data in the literature through large studies and functional testing to prove that variant is likely related to carrier status. Variants that are predicted based on protein function models or a few case reports to be deleterious are not reported.

All Other Sections Review

Filter for variants found in <40% of the population, are found in a public database that collates available research, and research indicates that the variant is associated with a trait or disease with a statistical significance of 10-8 or better in a population relevant to the person being tested. Exceptions are subjective and are made when functional studies prove the variant changes enzyme levels, or the research available provides meaningful information to the individual’s personal or family medical history. Although many variants are reviewed, this test is not diagnostic and may miss key variants in many genes. For example, large deletions and rearrangements will not be discovered.
